The Community Health Improvement Plan (CHIP)

The Community Health Improvement Plan, or CHIP, identifies priorities for improving the health and well-being of the Fairfax community over a five-year period. The Partnership for a Healthier Fairfax adopted its first CHIP in 2013 and has successfully implemented over 90% of the activities outlined.

Now the Partnership has launched its next five-year plan and is hoping to build on the strong foundation of collaboration and collective impact upon which the first CHIP was built.
